The General Practitioner or referring specialist remains the principal medical professional for the effective management of each patient’s clinical health status. However the complexity of care necessary for obese people with comorbidities and Type 2 Diabetes requires systematic care from a multi-disciplinary team of qualified health professionals, with the General Practitioner remaining as the central point of primary care.
Our programs are evidence-based, medically-supervised, and run by a team of qualified health professionals. You can be assured your patients will receive the highest level of patient care, and our commitment to improved patient outcomes – specifically by improving nutritional knowledge, weight loss, improved aerobic fitness and lean tissue gains.
Our clinicians will carefully assess each new patient, their adherence to their GP treatment strategy and recommend the type of program best suited to them. We expect each patient to be unique in their mix of requirements across weight loss, nutritional recommendations, activity and resistance exercise goals. For diabetic patients, we will also assess their current status of feet and eye care.
We will liaise with you at the commencement of each program, copy all blood pathology to you, provide EPC reporting and with final results on completion.
Our electronic practice management platform holds all the patient information for all team care providers so each team member is updated at all times with progress and results.
We accept Enhanced Primary Care (EPC) referrals. Medicare and Better Access Program rebates are also provided where patient eligibility exists.
Patients can also receive Private Health rebates on their consultations. Patients should be directed to their Private Health provider to discuss specific levels of cover available to them. At each appointment the appropriate receipt can be provided enabling any eligible rebates to be claimed.
We have created a Rebate Services Guide to give an indication of rebates for the most common private health providers. This is a guide only, clients/patients should contact their Private Health fund to determine the exact rebates they are eligible for.
